ZO Product Line
Dr. Zein Obagi, a Beverly Hills dermatologist, creator of the original Obagi Nuderm program has advanced beyond Nuderm with his ZO Anti-Aging and Therapeutic programs. Your needs are assessed and the program is tailored by Dr. Archer to accommodate your individual needs. ZO corrects and controls teen and adult acne, improves blotchy pigmentation, and stimulates new collagen & elastin production to smooth wrinkles and make skin younger looking. Dr. Archer initially evaluates, then after 2 and 8 weeks on the plan, she rechecks the patient's progress and adjusts the program to tailor it to the patient's response.

Collagen has been replaced by Juvederm and Restylane . these help to fill in lines around the mouth, from the nose to the mouth, plump the lip line, fill in certain acne scars, fill some forehead and frown creases. Not safe for use for laugh lines around the eyes (due to blood flow in this area, does have a low potential for embolus and blindness if injected in this area). These are both hyaluronic acid products. They do not require a test patch first. There is a very low incidence (under 3%) of adverse reactions at the injection site. The filler effect can be reversed with Wydase. There are other fillers available for different purposes, but Dr. Archer is not using them at this time.
Botulinum toxin was developed in the early 1980's by Dr. Scott, a San Francisco ophthalmologist, for temporary treatment of strabismus and blepharospasm. It causes temporary paralysis of muscles and certain sweat glands. It is used for abnormal spasm of eyelid, facial and neck muscles. Controls severe axillary sweating. Used cosmetically to relax facial lines and neck bands caused by muscle contraction. FDA approved for frown line improvement and severe axillary sweating, also effective for relaxing laugh lines, forehead lines (NOT an option if the patient has droopy eyebrows), prominent neck bands, "gummy" smile (lip raises too high and exposes upper gums when patient is smiling) and downturning corner of the mouth (relaxes the muscle that pulls the mouth corner downward).
"Functional" or medically necessary uses include blepharospasm, hemifacial spasm, neck spastic torticollis, and migraines.
Dr. Archer was trained in the proper use of Botox® in 1986, and continues to be one of Allergan's consultants in training other physicians in the proper usage and administration of Botox®
Dr. Obagi replaced his Blue Peel with his ZO Medical 3 step peel which is performed in office then followed with an at home treatment that produces longer lasting results than ordinary peels. It is dual action, providing epidermal renewal and dermal stimulation. Improves acne, melasma, sun damage, fine lines, texture roughness, large pores and dull appearing skin. It causes minimal burning & stinging, plus is suitable for any skin type or color. It utilizes a blend of exfoliants, 6% concentration of retinol, & multi-action agents to improve skin health & treat signs of skin aging. It supports the skin's barrier function, improves the skin's ability to repair & renew itself, provides anti-oxidant protection, DNA repair & is anti-inflammatory. The peel is a blend of salicylic acid, trichloracetic acid & lactic acid to remove the outermost layers of the skin. Saponins minimize the skin reaction and glycerin improves hydration. Prior to treatment, you must be on high potency retinols for 6 weeks. Step 2 includes the retinol crème, and step 3 is the calming crème post-procedure skin relief.
Dr. Archer uses a Physician strength machine, different than those available in spas. It uses fine granules of sodium biarbonate to essentially finely "sandblast" the skin's surface. This removes the outer surface layer of skin. You will notice immediately, that the skin feels smoother. Repeat treatments help stimulate new collagen production to make the skin look younger. Can also help with some blotchy pigmentation. For patients with acne, the treatment allows oxygen to penetrate into the lesion better which causes the lesion to resolve faster. Not for patients with severe acne breakouts. There is no "downtime", the patient may return to work or school immediately after treatment.
The Sciton resurfacing laser is used to ablate a minimal depth of skin; deeper than microdermabrasion, but nowhere near as deep as full laser skin resurfacing for the face. The result is that the patient looks and feels rather like they have a sunburn for 3-4 days. This treatment helps more than microdermabrasion with fine lines and pigmentation problems. it also stimulates more collagen production for youthful looking skin. This treatment can only be used on the face.
Non-ablative Skin Rejuvenation
This Sciton laser does not disrupt the skin's surface; instead, it works below the surface. Takes 3-4 treatments spaced a month apart. Full face treatment takes 45 to 50 minutes for Dr. Archer to perform. The patient is treated with a topical anesthetic for 30 minutes prior to treatment. This treatment can be used on a variety of skin colors and on a variety of body areas to tighten the skin.
Laser Skin Resurfacing
This Sciton laser ablates the skin to a certain level. It is most effective in improving deeper lines and wrinkles, but not guaranteed to eliminate all wrinkles. Requires anesthesia, either a local anesthetic for local treatment areas (around the eyes or mouth) or a "twilight" anesthesia for full face treatment, also requires pre and post treatment with oral antibiotics and antivirals. Initial recovery time is 10-14 days for complete regrowth of skin. This treatment is for the face only, not the neck. Safe only for paler skin types. Can not be used on Asian patients, as they tend to have uniform problems with abnormal pigmentation afterwards. There is a definate possibility of discomfort for the first few days after treatment.
Laser Vein Treatments
This Sciton laser can treat smaller superficial veins. Large varicose veins need different treatment (injections or surgical stripping). Treatments can be performed on legs, thighs, chest, and on certain areas of the face (the crease by the nose is a problem as there is a tendency for the laser to arc and cause a burn). More than one treatment may be necessary, and some vessels just don't respond well. Requires reduced physical activity (stopping exercise routine for 1 week) after treatment to prevent vessels from reopening. For legs or thighs, support stockings are recommended. No hot showers or saunas for 1 week following treatment.
Laser Hair Removal
This Sciton laser can treat most hair colors. However, extremely pale blond, gray and white hairs do not have enough pigment present to treat and get a response. This treatment reduces hair growth and either completely eliminates growth or may change growth to production of very fine, pale hairs. Usually takes 5-6 treatments spaced every 5-6 weeks to achieve the best results. If treatments are interrupted, even for 1-2 weeks off the cycle, then the count has to start over, due to natural hair growth. Can treat hair on certain facial areas such as upper lip, chin, along the jaw line, and sideburns. It is a tremendous help in folliculitis in darker skinned patients. Other areas that can be treated are the legs, thighs, arms, underarms, chest, back and bikini line. If the patient is interested in treating multiple areas, it may be necessary to divide the treatments into 2 sessions, as the laser cooling wand does warm up with extended use and requires up to 10 minutes of cool-down time in order to continue treatment.
The patient must shave 3-4 days before their appointment, not at the office immediately before treatment, as Dr. Archer needs to see a few days growth in order to guide treatment. If the patient has not shaved it will be necessary to reschedule the appointment, otherwise the laser energy is absorbed into the surface hair and not into the follicle and treatment is ineffective. Also, patient cannot epilate or wax prior to treatment.
